Meglitinide Analogues
Nateglinide, a nonsulfonylurea hypoglycaemic agent which stimulates insulin release from the pancreatic β-cells by blocking ATP-dependent K channels, depolarising the membrane and facilitating Ca entry through Ca channels. This action depends on the amount of existing glucose levels.
How to use Nateglinide:
Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Nateglinide is to be taken empty stomach.
Administration:
Take immediately before or up to ½ hr before meals.
Adult Dose:
Oral
Type 2 diabetes mellitus
Adult: 60 or 120 mg tid.
Increased levels/effects with enzyme inhibitors (e.g. fluconazole). Increased hypoglycaemic effects with salicylates, MAOIs, nonselective beta-blockers, alcohol, NSAIDs. Decreased levels/effects with enzyme inducers (e.g. rifampicin). Decreased hypoglycaemic effects with thiazide diuretics, corticosteroids, thyroid products and sympathomimetic agents.
Diabetic ketoacidosis; IDDM. Lactation.

Drugs should be given only if the potential benefit justifies the potential risk to the foetus.
Precaution:
Geriatric patients, debilitated and malnourished patients; adrenal or pituitary insufficiency, moderate to severe hepatic impairment; severe renal impairment. Monitor glycaemic levels during periods of stress. Pregnancy.
Lactation: unsafe
Symptoms: Hypoglycaemia. Management: Use IV glucose in severe reaction.
Store at 25° C.
